摘要:: 目的了解三级综合医院护士实施失能老人家庭照护者照护指导工作的体验，为促进优质护理资源下沉社区提供参考。方法对20名上海市某三甲综合医院参与失能老人家庭照护者照护指导工作的护士进行半结构式访谈，运用Colaizzi现象学资料7步分析法进行内容分析。结果共提炼出4个主题和8个类别：护理专业价值重构，涵盖家庭照护者照护能力提升带来成就感、职业价值和个人价值的体现和升华；角色拓展中的自我调适，涵盖护士角色延伸与专业胜任力挑战并存、非医院环境工作的顾虑与不适应；家庭情境下的沟通困境，涵盖护士与家庭照护者的健康信念冲突、家庭照护者情绪压力引发的沟通障碍；多重因素导致服务受限，涵盖时间不足与服务中断、缺乏多学科团队支持。结论应完善一体化培训体系，促进护士居家照护能力与健康教育能力协同提升；提升失能老人家庭照护者健康素养，化解双方沟通障碍；重视角色适应与心理调适，调动护士积极性；构建以三级医院护士为主导的失能老人家庭照护者照护指导模式，从而有效提升失能老人照护质量。

Abstract:: ObjectiveTo understand the experiences of nurses in tertiary general hospitals in providing care guidance to family caregivers of disabled elderly individuals, providing references for promoting the sinking of high-quality nursing resources into communities. MethodsSemi-structured interviews were conducted with 20 nurses from a tertiary grade-A general hospital in Shanghai who participated in care guidance for family caregivers of disabled elderly individuals. The content was analyzed using Colaizzi’s seven-step phenomenological data analysis method.ResultsFour themes and eight categories were identified: reconstruction of nursing professional value, encompassing the sense of achievement derived from enhancing the caregiving capabilities of family caregivers, as well as the manifestation and elevation of occupational and personal values; self-adjustment in role expansion, covering the coexistence of nurse role extension and challenges to professional competence, along with concerns and maladjustment to working in non-hospital environments; communication dilemmas in family contexts, including conflicts in health beliefs between nurses and family caregivers, and communication barriers arising from the emotional stress of family caregivers; service limitations caused by multiple factors, involving insufficient time and service interruptions, as well as a lack of multidisciplinary team support. Conclusion It is essential to improve an integrated training system to facilitate the coordinated enhancement of nurses’ home care capabilities and health education skills; elevate the health literacy of family caregivers of disabled elderly individuals to overcome communication barriers between both parties; emphasize role adaptation and psychological adjustment to mobilize nurses’ enthusiasm; and establish a nurse-led care guidance model for family caregivers of disabled elderly individuals in tertiary hospitals, thereby effectively improving the quality of care for disabled elderly individuals.
