摘要:
目的以成都市某三甲医院新农合联网即时结算的数据为例，探讨按病种付费的适应性选择，为医保支付方式改革提供数据支持与理论依据。方法利用模糊匹配法对出院诊断进行匹配，利用归一化法对病种的费用和住院天数进行评价，从而筛选适宜开展病种付费的病种。结果按照分级诊疗政策的分类，到三级医院就医的病种并非全为疑难急重症，内外科的适应性病种有所不同。结论管理部门在制定相关政策时应针对病种进行规定，在引导病人理性就医的同时提高医院管理水平和基金使用效率。
Abstract:
ObjectiveTo discuss the selection of adaptive diseases for DRGs-based payment system by taking the data of the new rural co-operative payment in a grade A tertiary level hospital in Chengdu as an example, and to provide data support and theoretical basis for the reform of the medical insurance payment based on the DRGs-based payment system.MethodsThe fuzzy matching method was used to match the discharge diagnosis, and the normalization method was used to evaluate the cost of the disease and the fluctuation of the length of stay in hospital, so as to select the adaptive diseases.ResultsThe medical treatment of diseases in accordance with the hierarchical medical system to the grade A tertiary level hospital were not all difficult and urgent cases, and the adaptability of different types of diseases were different. ConclusionPolicy makers should stipulate the specific diseases in order to guide the rational medical treatment of patients, which improves the level of hospital management and the efficiency of fund usage.
