目的调查我国三级公立医院出院患者随访工作开展现状,旨在为提升服务质量提供参考。方法基于第5次“全国改善医疗服务行动第三方评估”项目,对全国173家三级公立医院出院患者随访工作开展情况进行调查。结果参与调查的173家三级公立医院中,仅3家未开展出院患者随访工作。有72家(42.35%)设置了院级随访中心,90家(52.94%)为科级随访,均由院内医务人员实施;3家(1.77%)设置院级随访中心,并由第三方机构负责随访。不同类型医院患者随访形式有差异(P=0.006),肿瘤医院以“设置院级随访中心医务人员随访”为主,其余类型医院以“科室层面随访”为主。结论医院对组织开展患者随访工作持积极态度,但部分医院并未认识到患者随访的必要性和重要性。建议进一步提高对出院患者随访工作重视程度,结合自身实际、随访目的及患者需求,灵活选择或组合随访人员,从院级和科级层面建立多学科患者随访团队,并严格监管。
Abstract:
ObjectiveTo investigate the status of follow-up work of discharged patients in tertiary public hospitals in China to improve the quality of service.MethodsBased on the fifth "National Third-Party Evaluation of Actions to Improve Medical Service", a survey was conducted on the follow-up work of discharged patients in 173 tertiary public hospitals in China.ResultsAmong the 173 tertiary public hospitals in the survey, only 3 hospitals did not carry out follow-up work for discharged patients. 72 (42.35%) had set up hospital-level follow-up centers, and 90 (52.94%) were department-level follow-up. All follow-ups were carried out by in-hospital medical staff. The follow-up in three hospitals (1.77%) were responsible by third-party institutions after setting up a hospital-level follow-up center. Different types of hospitals had different methods of patient follow-up (P=0.006). Cancer hospitals mainly carried out follow up by medical staff in hospital-level follow-up center, while other types of hospitals mainly carried out follow up by department level.Conclusion Hospitals had a positive attitude towards organizing and carrying out patient follow-up work, but some hospitals did not realize the necessity and importance of patient follow-up. It is recommended that tertiary public hospitals further increase the importance of follow-up work for discharged patients, combine their own actual situation, follow-up purpose and patient needs, flexibly select or combine follow-up personnel, establish a multidisciplinary patient follow-up team at the hospital and department level, carry out strict supervision.
