医疗体制改革的目标之一是以患者为中心，提供连续性医疗卫生服务，优化医疗资源的配置和使用。基于质量效益，借鉴国外先进经验，以护理管理的视角，从信息协同、功能协同、技术协同、管理协同等4个方面设计系统的策略集，提出区域协同慢性病连续照护模式构想，为政策制定、分级诊疗提供参考。

One goal of medical system reform is to focus on patients, provide continuous medical and health services, and optimize the allocation and use of medical resources. Based on quality benefit and foreign advanced experience, from the perspective of nursing management, this study developed a system strategy set from four aspects of information collaboration, functional collaboration, technical collaboration, and management collaboration. A regional collaborative continuous care model for chronic diseases was expected for construction, which provided reference for policy formulation and hierarchical diagnosis and treatment.
