曼彻斯特患者安全框架在安全文化建设中应用的范围综述

【摘要】目的 综合分析曼彻斯特患者安全框架在安全文化建设中的应用现状，为医疗机构安全文化建设提供建议。方法 根据范围综述方法，系统检索中国知网、万方、PubMed、Embase、CINAHL、Web of Science、Scopus、ProQuest等中英文数据库，对建库至2023年7月8日收录的相关文献进行回顾分析。结果 共检索到286篇相关文献，最终纳入13篇。曼彻斯特患者安全框架的应用机构包含医院、社区药房、养老院、初级卫生保健机构等；实施对象主要为医生和护理人员；实施方法包含定性和定量两种。结论 曼彻斯特患者安全框架对改善安全文化有积极作用。未来需进一步探索曼彻斯特患者安全框架在不同人群、不同医疗机构中的应用，结合多种评估工具和干预方式，不断优化安全文化研究方案，促进医疗机构安全文化建设。

Abstract Objective To comprehensively analyze the application of the Manchester Patient Safety Framework in the construction of safety culture, and provide suggestions for the construction of safety culture in medical institutions.Methods According to the methodology of scope review, the databases of China National Knowledge Infrastructure, Wanfang, PubMed, Embase, CINAHL, Web of Science, Scopus, and ProQuest were systematically searched to review and analyze the relevant studies included from the establishment of the database to 8 July 2023.Results A total of 286 pieces of literature were retrieved and 13 were included. Applications of the Manchester Patient Safety Framework include hospitals, community pharmacies, nursing homes, primary health care facilities, etc. The target of implementation was mainly doctors and nursing staff. The implementation methods included qualitative and quantitative methods.Conclusion The Manchester Patient Safety Framework has a positive effect on improving safety culture. In the future, it is necessary to further explore the application of the Manchester Patient Safety Framework in different populations and healthcare organizations and to continuously optimize the research protocols of safety culture by combining multiple assessment tools and interventions, in order to promote the construction of safety culture in medical institutions.

文化是建立高可靠性组织的关键［1］。安全文化是组织与个人价值观、态度、能力和行为模式的产物，决定着组织对安全计划的承诺和执行方式［2］。美国卫生保健研究和质量机构（Agency for Healthcare Research and Quality, AHRQ）将安全文化评估作为医院十大患者安全策略之一［3］。评估医疗机构患者安全文化现状是明确安全文化发展阻碍因素并提出改善策略的首要步骤［4-5］。目前，安全文化研究以横断面调查为主，尚不能分析出组织内部安全文化深层次信息。曼彻斯特患者安全框架（Manchester Patient Safety Framework，MaPSaF）［6］是安全文化的质性研究工具，由英国曼彻斯特大学Parker D教授开发，用于评估初级保健组织的安全文化。MaPSaF是一种可靠的安全管理工具，能够改善患者安全［7］。本研究以Arksey H等［8］的范围综述方法为框架，对MaPSaF在医疗机构安全文化中的基本内容、应用机构等进行总结和分析，以期为今后MaPSaF在医疗机构安全文化研究中的应用提供参考。
1资料与方法
1.1明确研究问题
通过前期查阅文献，明确研究问题。具体审查问题包括：（1）MaPSaF研究内容是什么；（2）MaPSaF适用于哪些医疗机构；（3）MaPSaF应用效果如何。
1.2文献检索
采用自由词和主题词相结合的方式进行文献检索。以“Manchester Patient Safety Framework/MaPSaF”“ patient safety/safety management/ organizational culture”为英文检索词，检索PubMed、Embase、CINAHL、Web of Science、Scopus、ProQuest等英文数据库；以“曼彻斯特患者安全框架/曼彻斯特病人安全框架”“安全文化/患者安全文化/安全管理”为中文检索词，检索中国知网、万方等中文数据库。检索时限为建库至2023年7月8日。
1.3文献筛选
纳入标准：（1）主题涉及MaPSaF应用于安全文化建设的相关研究；（2）研究对象为参与MaPSaF研究的相关人员；（3）文献类型为描述性研究、质性研究、横断面研究、类实验研究或混合研究。排除标准：（1）重复发表、会议论文或具有相同数据源的文献；（2）非中、英文文献；（3）无法获得全文文献。由两名经过培训的研究人员将检索到的文献导入EndNote X9软件去除重复文献，然后阅读标题和摘要，根据文献纳入和排除标准进行初筛，再阅读全文进行复筛。若筛选过程中遇到分歧，则与第3名研究人员讨论决定是否纳入，参照Tricco AC等［9］形成的范围综述报告中PRISMA扩展版清单（PRISMA Extension for Scoping Reviews, PRISMA-ScR）进行范围审查，运用2016年澳大利亚JBI循证卫生保健中心质性研究质量评价工具［10］对文献整体质量进行评价，最终确定符合标准的文献。
1.4资料提取与分析
所有纳入的文献数据由两名研究人员使用标准化表格独立提取信息并进行整合，提取内容包括作者、年份、国家、研究类型、研究对象、研究主题等。
