三级肿瘤专科医院医疗质量综合评价指标体系构建

【摘要】目的 构建三级肿瘤专科医院医疗质量综合评价指标体系，为肿瘤专科医院医疗质量监测和持续改进提供工具。方法 通过文献梳理初步确定医疗质量评价指标池，运用德尔菲法展开两轮专家函询，建立评价指标体系，采用层次分析法赋予各级指标权重。结果 两轮专家函询的专家积极系数分别为85.18%、100%；专家权威系数分别为0.82、0.85；总体指标协调系数分别为0.364、0.372，P均＜0.05。构建的三级肿瘤专科医院医疗质量综合评价指标体系包括一级指标5项、二级指标55项。结论 构建的三级肿瘤专科医院医疗质量综合评价指标体系具有一定科学性，能够较好反映政策导向及医院发展需求，可为进一步丰富医疗质量评价内涵及助推医院高质量发展提供依据。

Abstract Objective To construct a comprehensive evaluation index system of medical quality in tertiary cancer hospitals, and to provide tools for monitoring and continuous improvement of medical quality in cancer hospitals.Methods The medical quality evaluation index pool was preliminarily determined through literature review, two rounds of expert correspondence consultation were conducted by Delphi method, an evaluation index system was established, and weights were assigned to indicators at all levels by analytic hierarchy process. Results The positive coefficients of the two rounds of expert correspondence were 85.18% and 100% respectively. The expert authority coefficients were 0.82 and 0.85, respectively. The overall index coordination coefficients were 0.364 and 0.372, respectively, P< 0.05. The comprehensive evaluation index system of medical quality in tertiary cancer hospitals included 5 first-level indexes and 55 second-level indexes.Conclusion The constructed comprehensive evaluation index system of medical quality of tertiary cancer hospitals is scientific, which can better reflect the policy orientation and the development needs of hospitals, and can provide a basis for further enriching the connotation of medical quality evaluation and promoting the high-quality development of hospitals.

医疗质量是医院核心竞争力的重要表现［1］。在国家公立医院绩效考核背景下，医疗质量评价不仅能帮助医疗机构了解自身医疗质量现状，发现存在问题，而且可以对标同类医疗机构，了解彼此差距，以便持续改进。目前，国内关于综合医院医疗质量的评价研究趋于成熟［2］，相较于综合医院，肿瘤专科医院因学科特点及患者群体特殊性，其医疗质量评价研究较少。本研究运用德尔菲法及层次分析法对三级肿瘤专科医院医疗质量综合评价指标展开研究，以期建立一套符合三级肿瘤专科医院医疗质量特点的综合评价指标体系，助力肿瘤专科医院高质量发展。

1 资料与方法

1.1文献检索
以“医疗质量”“评价指标”“评价体系”“medical quality”“evaluation index”“evaluation system”等为关键词，在CNKI、万方、PubMed等数据库检索2013年1月-2022年12月相关文献，排除非公立医院、非三级医院文献，共纳入26篇文献。通过文献分析，结合《国家三级公立医院绩效考核操作手册（2023版）》，初步构建包含5项一级指标、54项二级指标的医疗质量评价指标池。

1.2德尔菲法
以德尔菲法函询相关领域专家对指标的意见及建议。专家选取范围为江苏省所有三级肿瘤医院从事临床和（或）管理工作的相关专家，专家选择标准：（1）从事医院临床和（或）管理工作≥10 a；（2）中级及以上职称；（3）本科及以上学历；（4）具有较强的责任心，对本研究具有较高的积极性，能够客观、全面地提供意见及建议。
依据医疗质量评价指标池形成专家函询问卷，各级指标采用Likert 5级评分法进行评价，其中，很重要为5分，重要为4分，一般为3分，不重要为2分，很不重要为1分。本研究共开展两轮专家函询，第一轮函询结束后根据专家意见及建议进行指标调整与完善，形成第二轮专家函询问卷，并展开第二轮函询。专家意见集中程度用指标重要性评分均数、满分频率和变异系数（CV）表示，指标重要性评分均数越大、满分比越高、CV越小，说明专家意见越集中，指标重要性越高。为保证评价结果的一致性，两轮函询为同一组专家。
