社区居民电子健康档案使用障碍研究

【摘要】目的 对国内外社区居民电子健康档案使用障碍进行分析，为推动我国电子健康档案建设提供参考。方法 检索PubMed、Web of Science和中国生物医学文献数据库2013年1月1日-2021年2月1日相关文献，提取第一作者、年份、国家/地区、使用障碍等信息。结果 共纳入17篇文献。社区居民电子健康档案使用障碍包括个人障碍、隐私和安全、技术障碍和外界障碍共4类，其中，隐私和安全问题是出现频数最高的障碍类型，个人障碍是我国纳入文献中出现频数最高的障碍类型。结论 个人认知和一般社会人口学特征以及隐私和安全问题是阻碍我国社区居民使用电子健康档案的主要因素。建议我国相关部门多渠道、分人群宣传电子健康档案；完善居民隐私保护措施，提高居民隐私保护意识；建立统一的电子健康档案管理平台；健全“建档-管档-用档”体系。

Abstract Objective To review the barriers of using the Electronic Health Records (EHR) among domestic and foreign community residents, and to provide reference for promoting the construction of EHR in China.Methods PubMed, Web of Science and Chinese Biomedical Literature Database were searched for relevant literatures from January 1, 2013 to February 1, 2021, and information such as first author, year, country/region, and use barrier were extracted.Results A total of 17 studies were included. The barriers to the use of EHR for community residents included personal barriers, privacy and security barriers, technical barriers and external barriers. Among them, privacy and security problems were the most frequently encountered barriers, and personal barriers were the most frequently encountered barriers in the included literature.Conclusion Personal cognition and general social demographic characteristics as well as privacy and security issues were the main factors hindering the use of EHR by Chinese community residents. It was suggested that relevant departments in China promote EHR through multiple channels and population segmentation; improve residents' privacy protection measures and improve their awareness of privacy protection; establish a unified EHR management platform; improve the system of "file building, file management and file use".

《“健康中国2030”规划纲要》提出，到2030年，实现国家省市县四级人口健康信息平台互通共享，人人拥有规范化的电子健康档案和功能完备的健康卡，远程医疗覆盖省市县乡四级医疗卫生机构，全面实现人口健康信息规范管理，满足个性化服务和精准化医疗需求［1］。电子健康档案（Electronic Health Record, EHR）是收集居民在医疗护理过程中相关健康信息的电子化记录，这些信息包括个人基本信息、主要卫生服务记录、主要疾病和健康问题摘要等内容［2-3］。有研究［3］表明，应用EHR有利于降低医疗成本，减少医疗错误，促进人群健康发展。EHR是我国基本公共卫生服务的重要项目之一。我国EHR建设存在被建档、人户分离、建而未用、健康信息碎片化等问题［4-7］，这说明我国居民在接受和使用EHR上仍存在一定障碍。国内外研究也发现此类问题，但少有对社区居民EHR使用障碍类型进行分析的研究。本研究对社区居民EHR使用障碍进行范围审查，旨在为我国EHR建设提供参考。
