我国日间手术政策文件的可视化分析

【摘要】目的 了解我国日间手术相关政策文件的演变过程，为规范日间手术发展提供参考。方法 以“日间手术”为检索词，在中国政府网进行检索，时间截至2021年10月31日。利用DiVoMiner在线文本分析软件进行政策文件可视化分析。结果 共检索出49份日间手术政策文件，首份文件发布于2011年，2017年达到最高峰（11份）。发文机构除国务院外，涉及国家卫生健康委员会等14个中央政府部门。政策文件主题涉及医药管理、卫生、社会管理三方面；对日间手术的关注角度主要集中在医疗支付制度（29.82%）、医疗服务能力（26.31%）与医院管理能力（22.81%）等方面。结论 政府对日间手术经历了从认识、深入了解到顶层规划的过程，国家政策的适时调整促进了日间手术规范发展。日间手术落地需要政府层面的顶层设计、医疗供给方的全面支持以及患者与社会的认可。

Abstract Objective To investigate the evolution of policy documents related to ambulatory surgery in China, so as to provide reference for the standardized development of ambulatory surgery. Methods "Ambulatory surgery" was used as the search term on the Chinese government website as of October 31, 2021. Visualization analysis of policy documents was performed using DiVoMiner text analysis software.Results A total of 49 ambulatory surgery policy documents were retrieved. The first document was published in 2011,peaked in 2017 reaching 11. In addition to The State Council, 14 central government departments including the National Health Commission were involved. The themes of the policy documents cover three aspects: medical management, health and social management; the attention of ambulatory surgery mainly focused on the medical payment system (29.82%), medical service ability (26.31%) and hospital management ability (22.81%).Conclusion The government has experienced a process of understanding and in-depth understanding of ambulatory surgery to top-level planning, and the timely adjustment of national policies has promoted the standardized development of ambulatory surgery. The implementation of ambulatory surgery requires top-level design at the government level, comprehensive support from medical providers, and recognition from patients and the society.

日间手术起源于20世纪初的苏格兰小儿外科，现已发展成为一种重要的手术模式［1-2］。我国于21世纪初引入日间手术。经过20余年的发展，日间手术已得到了社会各界的认可。但截至2020年，全国日间手术占择期手术比例仍低于《关于印发开展三级医院日间手术试点工作方案的通知》（国卫医函〔2016〕306号）中“到2020年之前，日间手术占择期手术比例将达20%～30%”的目标。这提示日间手术在国内的发展仍存在一定阻力。国家政策对医疗卫生事业的发展有着重要影响。本研究对2021年10月31日之前我国政府发布的日间手术相关政策文件进行可视化分析，回顾日间手术政策演变过程，以期为进一步规范日间手术发展提供参考。
