提高高值医用耗材管理信息追溯覆盖率

【摘要】目的 提高高值医用耗材管理信息追溯覆盖率，确保耗材使用安全。方法 开展QC小组活动，通过现状调查、原因分析、要因确认等找出问题的主要原因，从建立统一标识编码、采用电子化仓储工具、建立统一追溯管理系统三方面进行改进。结果 高值医用耗材管理信息追溯覆盖率由64.3%提高到91.0%，达成了活动目标。结论 QC小组活动的开展，提高了医院手术室高值医用耗材管理信息追溯覆盖率，提升了高值医用耗材管理精细化水平。未来应形成循环机制，建立高值医用耗材管理长效机制。

Abstract Objective To improve the traceability coverage rate of hospital high-value medical consumables management and ensure the use safety of consumables.Methods QC group activities were carried out to find out the main causes of the problems through current situation investigation, cause analysis, and confirmation of major causes, and improvements were made from the establishment of unified identification and coding, the use of electronic storage tools, and the establishment of a unified traceability management system.Results The traceability coverage rate of management information of high-value medical consumables increased from 64.3% to 91.0%, achieving the activity target.Conclusion The implementation of QC group activities has improved the traceability coverage rate of high-value medical consumables management information in the Operating Room of the hospital and the refinement level of high-value medical consumables management. In the future, it is necessary to form a circulation mechanism and establish a long-term mechanism for the management of high-value medical consumables.

1选题理由

高值医用耗材作为严格管控的第三类耗材［1］，是指直接应用于人体，对安全性有严格要求，临床使用量大，价格较高，风险较高的医用耗材［2］。2019年，国务院印发《治理高值医用耗材改革方案》（国办发〔2019〕37号），启动了高值医用耗材综合性治理改革。提高高值医用耗材管理信息追溯覆盖率，对耗材改革及患者安全具有重要意义。
从天津市肿瘤医院耗材采购实际来看，医院年耗材采购金额约为8亿元，其中高值医用耗材占比约为40%。相关数据显示，高值医用耗材导致不良事件占该院全年不良事件上报总数的36.9%，严重伤害占6.7%。为此，该院成立QC小组，以提高高值医用耗材管理信息追溯覆盖率。
名词定义：高值医用耗材管理信息是指耗材采购、发放、使用、计费等各环节中产生的耗材名称、注册证号、采购数量、生产批号、产品序列号等信息。高值医用耗材管理信息追溯覆盖率即围绕高值医用耗材一个管理周期内，对于耗材管理信息追溯产生的项目数占可追溯总项目数的百分比［3］。

2现状调查

小组成员于2020年4月1日-7月30日，对医院高值医用耗材使用情况进行调查，结果显示，医院高值医用耗材种类为785种，手术室高值医用耗材种类达563种，占71.7%。设定每月随机抽取300项手术室高值医用耗材管理信息，经调研汇总，在2020年4月-7月共计1 200项手术室高值医用耗材管理信息中未追溯到项目多达428项，高值医用耗材管理信息追溯覆盖率仅为64.3%。
针对2020年4月-7月手术室高值医用耗材管理信息追溯未覆盖项目进一步展开调查，发现产品序列号信息追溯未覆盖项目达240项，占56.1%。产品序列号是产品追溯到患者的唯一生产代码，故小组成员将产品序列号追溯覆盖率低作为主要症结。
