摘要:
目的探索建立基于医联体的“专科-全科团队”（专全团队）模式，并进行效果评估。方法按照重要性、科学性、可行性指标遴选原则设计评估体系，对全科医师慢性病管理能力和社区卫生服务中心服务能力及分级诊疗作用进行评估。结果基于医联体的专全团队建设，使全科医师慢性病管理能力平均提高了60分，社区卫生服务中心服务能力平均提高了65分，双向转诊患者数量平均增加了34例。结论基于医联体的专全团队模式对全科医师慢性病管理能力和社区卫生服务中心医疗服务能力提升均有一定作用，对分级诊疗有一定促进作用。下一步可从制度、管理、个人层面持续改进。
Abstract:
ObjectiveTo explore the establishment of the "specialty-general practice team" mode based on medical association, and evaluate the effect.MethodsThe evaluation system was designed according to the selection principles of importance, science, and feasibility indicators to evaluate the chronic disease management ability of general practitioners, the serviceability of community health service centers, and the role of graded diagnosis and treatment.ResultsThe construction of specialty-general team based on medical association, the chronic disease management ability of general practitioners increased by 60 points on average, the service ability of community health service centers increased by 65 points on average, and the number of two-way referral patients increased by 34 cases on average.Conclusion The specialty-general team mode based on medical association has a certain effect on the improvement of chronic disease management ability of general practitioners and the medical service ability of community health service centers, and has a certain promotion effect on hierarchical diagnosis and treatment. In the future, it can be continuously improved from the system, management, and individual levels.
