摘要:
分析某口腔专科医院住院患者申请院外会诊的现状，为提高医疗质量与安全提出对策与建议。方法从医院住院电子病历系统中提取2017全年住院患者中申请院外会诊的所有相关数据，从受邀会诊科目、介入时间、性别与年龄构成、主要病种、会诊原因等维度进行统计分析。结果院外会诊比例达12.6%。会诊科室中的临床内科比例达74.53%，会诊医师术前参与诊疗比例达93.74%。会诊的主要原因是心功能异常、肺功能异常、血糖异常、高血压、既往脑病史等。会诊患者中以中老年为主，且男性显著高于女性。各类会诊疾病中口腔颌面部良恶性肿瘤的会诊比例为70.62%。同时重点口腔疾病的治愈好转率达到较高水平。结论口腔专科医院邀请院外会诊主要针对各类口腔肿瘤患者，参与术前基础疾病控制，指导围手术期治疗。医院应关注会诊患者安全，规范内部会诊制度与流程，细化住院前基础疾病的预防与筛查，与综合性医院开展医联体，提供远程医疗服务，提升医疗质量，以有效降低法律风险。
Abstract:
To analyze the status of out of hospital consultation in a stomatological hospital to provide suggestions on the improvement of medical quality and safety.MethodsExtracting all out of hospital consultation relevant data in 2017 from hospital inpatient electronic medical records system, and conducting statistical analysis from aspects of subjects, time, gender and age, main diseases types, consultation reasons, etc.ResultsThe proportion of out of hospital consultation was up to 12.6%. The proportion of clinical internal medicine was 74.53% among all the consultation departments, and the preoperative application rate reached 93.74%. The main reasons for consultation were abnormal heart function, abnormal lung function, abnormal blood glucose, hypertension, and previous history of encephalopathy etc. Among all consultation, the consultation rate of tumors in oral and maxillofacial was 70.62%, the consultation patients were mainly middle aged and elderly,and it was significantly higher in males than in females.ConclusionThe out of hospital consultation in the stomatological hospital was mainly for patients with various types of oral tumors, participating in preoperative basic disease control and guiding perioperative treatment. Hospital should pay attention to the safety of consultation patients, optimize consultation procedures, refining the prevention and screening of pre-hospital underlying diseases,integrate with comprehensive hospitals to provide telemedicine services, improve the quality of medical treatment to reduce legal risks effectively.
