摘要:
目的分析2014年-2016年我国三级医院呼吸机通气总日数及VAP发病率现状,为完善VAP相关研究提供依据。方法从国家护理质量数据平台获取2014年-2016年呼吸机肺炎相关数据。采用Excel 2016、Stata 14.1软件以及固定效应模型分析机械通气总日数和呼吸机相关性肺炎发生率随时间的变化情况。结果ICU呼吸机通气总日数逐年增加,2015年和2016年分别较2014年增加324天和1 028天(P<0.01);VAP发生率逐年下降,2015年和2016年分别较2014年下降0.4‰和1.5‰(P=0.018)。结论随着呼吸机的广泛使用,呼吸机通气总日数逐年增加,VAP发生率逐年下降,这与国家政策支持密不可分,未来应进一步完善管理,改进预防策略,以降低VAP发病率。
Abstract:
ObjectiveTo analyze the total days of mechanical ventilation and incidence of ventilator-associated pneumonia (VAP) in tertiary hospitals in China from 2014 to 2016, and to provide evidence for improving VAP related studies.MethodsThe data of VAP was obtained from the National Nursing Quality Data Platform from 2014 to 2016.Excel 2016 and Stata 14.1 software,and a fixed effect model was used to analyze the days of mechanical ventilation and the incidence of VAP.ResultsThe days of ICU mechanical ventilation increased by year. Compared with that in 2014, the total number of days increased 324 days and 1028 days in 2015 and 2016(P<0.01). The incidence of VAP decreased by year, and compared with that in 2014, the incidence of VAP decreased by 0.4‰ and 1.5‰ in 2015 and 2016 (P=0.018). Conclusion With the widespread use of ventilators, the days of mechanical ventilation was increasing while the incidence of VAP in Chinese tertiary hospitals was decreasing year by year, which was closely related to the increasingly tendencies of national policies. In the future, management and improvement strategies should be further improved in order to reduce the incidence of VAP.
