摘要:
目的分析护理人力对住院患者跌倒的影响,为合理配置护理人力资源提供参考。方法利用国家护理质量数据平台421家三级甲等综合医院2014年-2016年的资料构建面板数据,运用随机效应模型对数据进行分析。结果每位责任护士负责照护患者数、护士离职率、医院床位数对住院患者跌倒发生率有显著正向影响。平均每名护士负责照护患者数在5名~7名,比照护患者数低于5名时的患者跌倒风险高0.35%~0.45%。护士离职率从1%增加至2%~4%时,患者跌倒风险增加0.3%以上;护士离职率增加至4%及以上时,跌倒风险增加0.66%。结论合理控制医院护患比,降低护士离职率,有助于降低住院患者跌倒风险。
Abstract:
ObjectiveTo measure the relationship between nursing manpower and risk of fall among patients, and to provide evidence for reasonable allocation of human resources.MethodsThe panel data of nurse staff and patient fall information from 421 tertiary general hospitals in the National Database of Nursing Quality during 2014 to 2016 was used for analysis by using random effect model. ResultsThe number of patients in charge by each responsible nurse, the demission rate of nurses and the number of hospital beds had a significant positive effect on the incidence of falls among inpatients. Compared with the hospitals where each nurse take care of 5 patients or less, the average risk of patient falls was 0.35%-0.45% higher when the number increased to 5-7. The average risk of patient falls was 0.3% higher with nurse demission rate of 2%-4% compared with that of 1%; and the risk was 0.66% higher with nurse demission rate of 4% or more. Conclusion It is an effective way to reduce hospital patient falls by controlling nurse-to-patient ratio and reducing nurse demission rate.
