摘要:
目的探讨基于医疗失效模式与效应分析（HFMEA）的清单式管理在软式内镜清洗质量改进中的应用效果。方法采用系统随机抽样法，选取2022年7月1日-12月31日上海某三甲医院内镜中心260条软式内镜作为对照组，选取2023年1月1日-6月30日252条软式内镜作为实验组。对照组按照《软式内镜清洗消毒技术规范》要求执行手工清洗；实验组通过HFMEA优化工作流程，针对高危环节设计可视化清单。结果改进后，软式内镜清洗的关键失效模式的RPN值大幅度下降；与对照组比较，实验组软式内镜清洗合格率、重复清洗率均得以改善。结论基于HFMEA的清单式管理通过空间定位与行为引导相结合，可以有效降低软式内镜清洗过程中的失效风险，也为软式内镜感控质量提升提供了参考。
Abstract:
ObjectiveTo investigate the application effect of list management grounded in Healthcare Failure Mode and Effect Analysis (HFMEA) in improving the cleaning quality of flexible endoscopes.MethodsUsing a systematic random sampling method, 260 flexible endoscopes from the endoscopy center of a tertiary hospital in Shanghai from July 1, 2022, to December 31, 2022, were selected as the control group, and 252 flexible endoscopes from January 1, 2023, to June 30, 2023, were selected as the experimental group. The control group performed manual cleaning in accordance with the requirements of the Technical Specifications for Cleaning and Disinfection of Flexible Endoscopes; the experimental group optimized the workflow through HFMEA and designed a visual checklist targeting high-risk steps.ResultsAfter the improvement, the Risk Priority Number (RPN) values of critical failure modes in flexible endoscopes cleaning significantly decreased. Compared with the control group, the qualified rate of flexible endoscopes cleaning and the re-cleaning rate in the experimental group were both improved.Conclusion List management based on HFMEA, through a combination of spatial localization and behavioral guidance, can effectively reduce the risk of failure during the cleaning process of flexible endoscopes and provides a reference for enhancing infection control quality in flexible endoscopes management.
