摘要:
目的了解全国产科专业医疗服务质量现状，为产科质量持续改进提供参考。方法基于国家医疗质量管理与控制信息网，收集并分析2019年度全国31个省、自治区、直辖市（含新疆生产建设兵团）共4 730家医疗机构产科专业医疗相关数据。利用SPSS 20.0软件对所得数据进行分析。结果2019年，我国剖宫产率为43.40%，初产妇剖宫产率为37.68%，民营医院剖宫产率较高；足月新生儿5分钟Apgar评分<7分发生率为0.77%，≥28周死胎发生率为0.36%，均呈现西高东低特征；阴道分娩椎管内麻醉使用率和严重产后出血发生率总体呈上升趋势，各地区具有差异性。结论我国产科医疗服务质量存在地区和机构类别差异。医疗机构应持续关注产科过程质量和结果质量。国家产科专业医疗质量控制中心应开展针对性质控，助推产科服务同质化发展。
Abstract:
ObjectiveTo investigate the current situation of obstetric professional medical service quality in China and provide reference for the continuous improvement of obstetric quality.MethodsBased on the National clinical Improvement System, medical safety data related to obstetrics in a total of 4 730 medical institutions in 31 provinces, autonomous regions and municipalities directly under the Central Government (including Xinjiang Production and Construction Corps) were collected and analyzed in 2019. The SPSS 20.0 software was used to analyze the data.ResultsIn 2019, the cesarean section rate in China was 43.40%, 37.68% for primipara women, and private hospitals had a high cesarean section rate. The rate of 5-minute Apgar score < 7 was 0.77% in full-term newborns and the rate of stillbirth was 0.36% in pregnancies with gestational age of ≥ 28 weeks, which all showed characteristic of low in east area and high in west area. The rate of neuraxial anesthesia for vaginal delivery use and severe postpartum hemorrhage showed an overall upward trend with differences among regions. Conclusion There were regional and institutional differences in the quality of obstetric medical services in China. The quality of obstetric procedures and outcomes should continue to be a concern of medical institutions. The National Center for Healthcare Quality Management in Obstetrics should carry out targeted quality control to boost the homogenization development of obstetrics services.
