摘要:
目的调查全国门诊专业医疗质量现状,为加强全国门诊质量管理提供依据。方法收集并分析了2018年全国4 450家综合医院门诊相关的人力资源、空间资源、门诊服务、门诊病历、门诊诊断等质控指标情况。结果调查医院中,71.82%的医院施行实名制预约挂号,平均预约途径数为4.45个,医院MDT门诊开展率22.00%,医院门诊电子病历使用率为49.96%;医院门诊标准诊断库使用率为56.27%。委属委管医院、三级医院、二级医院的门诊诊断与出院诊断的亚目符合率分别为69.4%、62.9%、71.1%。结论医院应当加强预约挂号管理,提供多种预约挂号管理,提供多种预约挂号方式;积极开展多学科(MDT)门诊,为肿瘤等疾病患者提供个性化的诊疗方案;加快推进门诊电子病历使用;提高门诊诊断与出院诊断符合率。调查医院对门诊质量指标的理解存在差异,应当制定统一的门诊质控指标及标准,推行统一的门诊诊断编码库。
Abstract:
ObjectiveTo investigate the current situation of national outpatient medical quality and to provide the basis for strengthening the quality management of national outpatient.MethodsThe quality control indicators related to the outpatient clinics, such as human resources, space resources, outpatient services, outpatient records, and outpatient diagnoses in 4 450 general hospitals nationwide in 2018 were collected and analyzed.ResultsAmong the hospitals surveyed, 71.82% of them implemented real-name appointment registration.The average number of appointment channels in appointment registration hospitals was 4.45, and the MDT outpatient service rate was 22.00%.The utilization rate of outpatient electronic medical records was 49.96%. The utilization rate of standard diagnosis library in outpatient department was 56.27%.The concordance rate of outpatient diagnosis and discharge diagnosis in hospitals affiliated to the National Health Commission, tertiary hospitals and secondary hospitals were 69.4%, 62.9% and 71.1%, respectively. Conclusion Hospitals should strengthen the management of appointment registration and provide multiple appointment registration channels.Hospitals should actively carry out multidisciplinary team (MDT) outpatient service to provide personalized diagnosis and treatment plans for cancer and other patients.Hospitals should accelerate the use of outpatient electronic medical records.Hospitals should improve the concordance rate of outpatient diagnosis and discharge diagnosis.Differences in interpreting of outpatient quality indicators among hospitals enrolled were detected, it is necessary to formulate unified outpatient quality control indicators and standards, and implement unified outpatient diagnosis coding library.
