目的探究精神疾病患者住院总费用的影响因素，为控制住院医疗费用和提高精神专科医院住院医疗结局质量提供参考。方法利用单因素分析和多元线性回归分析，抓取北京市3家三级精神专科医院2016年1月1日-6月1日住院患者的病案首页数据进行统计分析。结果共纳入6 529条患者入院记录，住院总费用中位数为17 772.7元。除患者社会人口学因素、住院天数和诊断类型外，患者共患躯体疾病、是否接受电休克疗法和就诊医院对住院总费用均有显著影响。结论应重视精神疾病患者的共患躯体疾病预防和诊治，治疗方法和就诊医院的机构差异对住院总费用的影响值得进一步研究。
Abstract:
ObjectiveTo identify factors influencing the hospitalization expenses of psychiatry patients,and to provide a reference of controlling hospitalization expenditure and improving the quality of inpatient medical outcomes in psychiatric specialized hospitals.MethodsFront pages of inpatient medical records were extracted from three tertiary psychiatric hospitals from January 1 to June 1 2016 in Beijing, China and analyzed using univariate analysis and multiple linear regression.�?倓ResultsA total of 6 529 inpatient records were included with a median hospital expense of 17 772.7 Yuan. In addition to the patients’ socio-demographic factors, length of stay and type of diagnosis, patients’ physical comorbidities, utilization of electroconvulsive therapy, and provider were significantly associated with inpatients expenses. Conclusion Attention should be paid to the prevention and treatment of comorbid physical diseases in patients with mental illness. Further study may focus on the influence of treatment patterns and institutional differences of the hospitals on the total cost of hospitalization.
