摘要:
目的开发一种红细胞格局谱抗体分析软件并应用于临床,提升红细胞意外抗体鉴定的准确性、可溯源性及鉴定效率。方法通过计算机程序编程开发红细胞格局谱抗体分析软件,并应用于某院333例患者,对红细胞意外抗体进行鉴定和分析,并与人工判读结果进行比较。结果在对333例红细胞意外抗体筛查阳性患者进行抗体鉴定时,人工一次判读与软件判读均鉴定出特异性抗体290例、未能分类抗体28例。软件进一步鉴定出1例抗-C、e合并抗-Jka抗体,1例抗-E合并抗-Lea抗体及10例可疑特异性抗体,以上抗体在人工一次判读中均未被识别。软件判读与人工一次判读结果的一致性Kappa值为0.793,相符率为95.5%,与人工二次判读结果的一致性Kappa值为0.958,相符率为99.1%。软件判读时间中位数为22(20~24)s,显著短于人工判读时间中位数364(353~385)s,差异有统计学意义(P<0.001)。结论开发的红细胞格局谱抗体分析软件,可以更好地保障患者输血安全,结合人工判读能够进一步提升红细胞意外抗体检测的准确性,同时提高红细胞意外抗体检测效率,提升实验过程的可溯源性,具有较好的临床应用价值。
Abstract:
ObjectiveTo develop an antibody analysis software for erythrocyte pattern spectrum and apply it to clinical practice,so as to improve the accuracy, traceability and identification efficiency of unexpected antibody identification of red blood cell.MethodsThe antibody analysis software of red blood cell pattern spectrum was developed by computer programming and applied to 333 patients in a hospital. The unexpected antibody of red blood cell were identified and analyzed, and the results were compared with those of manual interpretation.ResultsIn the antibody identification of 333 patients with positive red blood cell unexpected antibody screening, 290 cases of specific antibodies and 28 cases of unclassified antibodies were identified by both manual interpretation and software interpretation. The software further identified one case of anti-C and e combined with anti-Jka antibody, one case of anti-E combined with anti-Lea antibody and 10 cases of suspected specific antibody, which were not identified in the initial manual interpretation. The Kappa value of the consistency test between the software interpretation and the initial manual interpretation result was 0.793, and the coincidence rate was 95.5 %. The Kappa value between the software interpretation and the secondary manual interpretation result was 0.958, with a coincidence rate of 99.1 %. The median interpretation time of software was 22 (20~24) s, which was significantly shorter than the median manual interpretation time of 364 (353~385) s, and the difference was statistically significant (P<0.001).Conclusion The developed red blood cell pattern spectrum antibody analysis software can better ensure the safety of blood transfusion in patients. Combined with manual interpretation, it can further improve the accuracy as well as efficiency of red blood cell unexpected antibody detection, and improve the traceability of the experimental process. It has good clinical application value.
