摘要:
针对1例LASA药品用药错误案例,分析用药错误原因，制定LASA药品规范管理制度，开展医务人员LASA药品用药安全培训，优化HIS功能，规范工作流程等改进措施并实施。对医院提出了健全LASA药品管理制度，优化医 院信息系统，营造患者安全文化氛围等建议；对医务人员提出了积极参与合理用药专项培训，学习LASA药品相关知识，制定患者参与用药安全策略等建议；对患者提出了树立用药安全意识，主动参与查对等建议；同时建议药 品监管机构、药品生产商、行业学会从源头规范LASA药品管理模式，以期减少LASA药品用药差错，确保用药安全。
Abstract:
Based on one case of Look-Alike and Sound-Alike(LASA) medication error, the causes were analyzed, the standardized management system of LASA medications was formulated, the safety training of LASA medication use for medical personnel was carried out, HIS function was optimized, workflow was standardized and other improvement measures were implemented. Suggestions were put forward to improve LASA drug management system, optimize hospital information system, and create a patient safety culture atmosphere. Furthermore, suggestions were put forward for medical staff to actively participate in the special training on rational drug use, learn LASA drug related knowledge, and formulate the safety strategy for patients to participate in drug use. The suggestions of establishing drug safety consciousness and taking part in checking were put forward for patients. It was suggested that drug regulatory authorities, drug manufacturers and industry institutes should regulate LASA drug management model from the source. These measures aimed to reduce LASA drug medication errors and ensure medication safety.
