【摘要】在新的历史时期,国家对公立医院提出了新的要求,公立医院的高质量发展迈进崭新阶段。文章以北京大学第三医院为例,介绍了该院以绩效管理为抓手,推动医院高质量发展的实践经验。文章阐述了该院的绩效考核原则和整体思路,以及该院在工作效率考核激励、综合考核激励、增量考核激励、特殊津贴等方面的精准激励措施,并进行了成效总结及思考,以期为医院管理者提供借鉴。

Abstract In the new historical period, the country proposed to the public hospitals new requirements and the high-quality development of public hospitals has entered a new stage. Taking the Peking University Third Hospital as an example, this paper introduced the practical experience of promoting the high-quality development of hospital with performance management. In order to provide reference for hospital managers, this paper expounded the principle and the general strategy of performance appraisal, as well as a series of accurate incentive measures in work efficiency assessment, comprehensive assessment,incremental assessment, special allowance, etc. Furthermore, the results were summarized and think.

1 我国公立医院发展现状

目前,我国社会城镇化及人口老龄化程度不断加深,人民群众的医疗服务需求快速增长。然而,医疗体制机制改革相对滞后,医疗资源配置长期不均衡,导致大型公立医院一方面服务人次持续增长,规模持续扩大,医院陷入粗放式规模发展的恶性循环[1];另一方面,由于医院规模的边际效益递减,大型公立医院规模发展的负效应日益凸显,主要表现为医院债务和医保垫付压力巨大、医护人员职业倦怠、医院风险防控削弱等[2]。患者的过度集中也引起资源不足与浪费并存、服务效率低下与社会矛盾加剧等一系列社会问题[3]。同时,医改逐渐进入深水区带来医疗环境的重大变化,使得医院原有的经营受到严重冲击,运营压力持续增加,原来重规模、轻内涵的粗放管理已难以适应新形势。
