【摘要】国家“十三五”深化医药卫生体制改革规划明确将分级诊疗作为重点工作任务，医师多点执业作为推进分级诊疗的重要措施近年来虽然备受关注，但其实施却困难重重，互联网医疗的快速发展有望为医师多点执业带来新的改变。文章在深入分析当前医师多点执业发展现状与挑战的基础上，系统梳理出了互联网医疗背景下医师多点执业呈现出的新形势以及存在的新问题，并从行业监管、机制设计、平台建设和服务模式等方面提出了对策建议，力求为医师多点执业的规范实施提供参考，加快推进分级诊疗政策落地。

Abstract The 13th Five-Year Plan of deepening the medical and health system reform has explicitly made the hierarchical medical system a key task. Although doctors' multi-sited license has become a hot issue on promoting hierarchical medical system in recent years, its implementation is still difficult. The rapid development of internet medical care is expected to bring new changes to the multi-sited license of doctors. According to in-depth analysis of the status quo and challenges on doctors' multi-sited license, we systemized the new situation and problems of doctors' multi-sited license by internet medical care, and put forward some countermeasures and suggestions from aspects of industry supervision, mechanism design, platform construction and service mode etc. It aimed to provide a reference for standardized implementation of doctors' multi-sited license, and to accelerate the implementation of hierarchical medical system.

2009年，我国政府首次将医师多点执业提上改革议程，提出要在规范审批的基础上积极稳妥开展多点执业，促进优质医疗资源的下沉与共享。2016年，国务院发布《“十三五”深化医药卫生体制改革规划》，将分级诊疗作为国家“十三五”期间的重点工作任务，同时把医师的多点执业作为推进分级诊疗的重要抓手。2017年，原国家卫生计生委公布《医师执业注册管理办法》，明确了医师区域注册制度，允许医师可在多家医疗机构同时进行执业［1］。国家卫生部门公布的数据显示，我国80%的医疗资源集中在大城市，其中有80%的医疗资源集中在大城市的大医院［2］，推进医师多点执业是优化医疗资源配置，推动医疗卫生事业快速发展的重要举措，但从目前医师多点执业实际实施的情况来看，效果并不理想，其推广还是存在很多困难，对医院管理也带来了诸多挑战。特别是随着互联网医疗浪潮的兴起，医师多点执业呈现出许多新的形态，也出现了许多新的问题，其中机遇与挑战并存。如何通过互联网医疗促进医师多点执业规范开展，让优质医疗资源下沉、高水平医生合理流动、平衡医疗卫生资源分布，将成为今后分级诊疗工作的重点与难点。本文着重从行业监管、机制设计、平台建设和服务模式等方面入手，研究互联网医疗对医师多点执业的作用与影响，并提出有关的对策及建议，为推动医师多点执业的规范开展提供借鉴与参考，推进分级诊疗政策加速落地。
