构建医联体平台下急性胸痛患者智慧化管理模式

【摘要】从复旦大学附属中山医院总院、紧密型医联体医院及患者3方面,分析急性胸痛患者管理现状。针对急性胸痛患者病史采集规范性、正确判断率、诊治流程等多个攻坚点拟定对策并加以筛选,形成3大方策群组予以实施:(1)建立急性胸痛规范化、同质化诊治流程;(2)基于大数据构建胸痛智慧化疾病诊断系统;(3)依托医联体增强胸痛患者早期预警意识。通过对策实施,形成了多项胸痛诊治标准流程,建立了胸痛诊断数据模型,为后续实践提供了参考。

Abstract This paper analyzed the management status of patients with acute chest pain from three aspects: the general hospital of Zhongshan Hospital affiliated to Fudan University, the close medical consortium hospital and the patients. Based on several key points, such as normalization of history collection of patients with acute chest pain, diagnostic accuracy, diagnosis and treatment process, countermeasures were developed and screened to form three policy groups for implementation: (1) establishment of standardized and homogeneous diagnosis and treatment process of acute chest pain;(2) establishment of an intelligent disease diagnosis system for chest pain based on big data;(3) relying on the medical consortium to enhance the awareness of early warning of patients with chest pain. Through the implementation of countermeasures, the standard procedures of diagnosis and treatment of chest pain were determined, and the diagnostic data model of chest pain was established, which provided reference for the subsequent practice.

选题背景:《健康中国行动(2019-2030)》明确指出,心脑血管疾病已成为居民的主要死亡原因和疾病负担,是制约健康预期寿命的重要因素[2]。原国家卫生计生委多次发文要求重视胸痛相关疾病的医疗救治工作,创新急诊急救服务[3-4]。复旦大学附属中山医院于2017年4月建立胸痛中心,构建了多部门协同的胸痛中心平台,取得了一定成效,但在急诊资源整合与调度、信息化平台建设等方面仍存在一定问题。国外文献报道显示,智慧医疗、人工智能机器人等可用于急救医疗[5-6];国内也有文献报道将医联体与智慧医疗相结合,应用于分级诊疗和卒中管理等[7-8]。本期活动拟探索在医联体平台下运用信息技术等对急性胸痛患者进行管理,帮助早期识别高危胸痛患者,降低漏诊误诊率,确保患者获得快速、优质的诊疗服务,提高胸痛管理水平。
