摘要:
减少白细胞输入是预防输血不良反应的关键。使用白细胞滤器(LDF)过滤是一种高效去除血液制剂中白细胞的技术。在我国,基于LDF的白细胞滤除技术在临床应用尚不广泛。为科学指导去白细胞血液在我国的合理应用,基于系统性文献检索和循证医学方法,采用Delphi专家共识法,对于LDF去白细胞的适用人群、人员资质和环境设施、使用时机、质量控制和管理等形成了专家推荐意见。指出,各医疗机构应根据本地区经济水平、输血服务能力、服务对象疾病谱等选择性针对某些目标人群开展白细胞滤除技术,鼓励经济水平高、输血服务能力强的地区或医疗机构普遍开展白细胞滤除技术。本共识可为白细胞滤除技术在输血治疗中的临床应用提供指导和参考,以确保临床用血安全。
Abstract:
Reducing the input of white blood cells is crucial for preventing adverse reactions to blood transfusion. The use of the Leukocyte Depletion Filter (LDF) for filtration is an efficient technique for removing white blood cells from blood products. In China, the clinical application of leukocyte depletion technology based on LDF is not yet widespread. To provide scientific guidance for the rational use of leukocyte-depleted blood in China, based on systematic literature retrieval and evidence-based medicine methods, the Delphi expert consensus method was employed to form expert recommendations regarding the applicable population, personnel qualifications and environmental facilities, timing of use, quality control, and management of LDF leukocyte depletion. It is pointed out that medical institutions should selectively implement leukocyte depletion technology for certain target populations based on local economic levels, blood transfusion service capabilities, and the disease spectrum of service recipients. Regions or medical institutions with high economic levels and strong blood transfusion service capabilities are encouraged to widely implement leukocyte depletion technology. This consensus can provide guidance and reference for the clinical application of leukocyte depletion technology in blood transfusion therapy, ensuring the safety of clinical blood use.
