摘要:
目的构建适用于县级医院的临床专科能力评估指标体系，为基层医疗机构专科能力建设提供参考。方法检索相关文献，参考“国家临床专科能力评估通用指标”进行指标初步分类和筛选；运用德尔菲法优化指标体系，层次分析法确定指标权重；通过赋分计算某县级医院32个临床科室专科能力水平。结果两轮咨询专家积极系数均为100%，权威系数介于0.795 ~ 0.995，协调系数从0.369增加至0.443（P均<0.05）。构建的指标体系包含服务能力、技术能力、质量安全、服务效率、持续发展5个一级指标，下设16个二级指标和43个三级指标，各级指标CR值均<0.1。体系应用后，评估结果能够客观反映样本医院临床专科能力水平。结论所构建评估指标体系适用于评价县级医院临床专科能力，可为县级医院高质量发展提供基线调查工具和后期效果量化评估工具。
Abstract:
ObjectiveTo construct a clinical specialty capability evaluation idex system applicable to county-level hospitals, providing reference for the clinical specialty capacity building of primary medical institutions.MethodsA literature analysis was conducted to retrieve relevant documents, and preliminary classification and screening of indicators were performed with reference to the "General Indicators for National Clinical Specialty Capacity Assessment". The Delphi method was employed to optimize the indicator system, and the Analytic Hierarchy Process was used to determine the weights of the indicators. The specialty capability levels of 32 clinical departments in a county-level hospital were calculated through scoring.ResultsThe positive coefficients of the two rounds of Delphi experts were both 100%, with authority coefficients ranging from 0.795 to 0.995, and coordination coefficients increasing from 0.369 to 0.443, all with P< 0.05. The constructed indicator system comprises 5 first-level indicators,including service capability, technical capability, quality and safety, service efficiency, and sustainable developmen, as well as 16 second-level indicators, and 43 third-level indicators. The CR values for all levels of indicators are less than 0.1. After application, the evaluation results objectively reflect the specialty capability levels of the sample hospital.Conclusion The constructed evaluation system is suitable for assessing the specialty capabilities of county-level hospitals and can provide a baseline survey and quantitative evaluation tool for later effects, contributing to the high-quality development of county-level hospitals.
